The OUHP-Tulsa Clinical Information Systems Account Request service allows OU Health Physicians faculty and staff to complete a role-based access form which will grant access to various medical applications. When filling-out the form, you may choose to request a new account, or modify/remove access for an existing user.
The person submitting this form MUST be the user's supervisor, manager, clinic manager, business administrator, or other applicable leader.
