Why is my OU Affiliate Account disabled?

In response to potential issues with federal compliance and cybersecurity concerns, OU Affiliate accounts were disabled on April 10, 2025 when the account met either of the two (2) following conditions:

  1. The account holder failed to complete the Affiliate Survey that was provided to them via email and their supervisor 5 times over a 2-week period.
  2. The account password was expired.

How do I get access to my OU Affiliate Account?

Please contact your supervisor and inform them they will need to complete the following Qualtrics Survey on your behalf. The survey response will be reviewed by appropriate parties and the account will be re-enabled (if deemed appropriate).

Please note: It may take up to 5 business days for the request to be reviewed, by non-OU IT parties, and a determination on the the account standing.

What type of questions does the survey ask?

You will need to work with your supervisor to assist them in answer the following questions related to your account:

  • All organizations (employers, universities, institutions, agencies, etc) which you are associated
  • Purpose of the OU account access
  • Purpose for the work (e.g. project, grant, collaboration, other)
  • Type of data you access
  • Country of residence
  • Current location
  • Country(ies) in which you perform work
  • Do you publish, share, or make available to third parties any data with individuals outside of the US
  • Identify all countries in which you are a citizen and/or have a visa or similar immigration status
  • Planning to travel to one of the following countries in the next 12 months (China - including Hong Kong and Macau, Cuba, Iran, North Korea, Russia, Venezula)
  • Access to certain data types (geolocation data, biometric data, human genomic data, personal health data, personal financial data)
