Restricted Request Forms

Some Services and request forms have restricted permissions limiting access to groups of authorized users. These Services include:

  • HSC Sponsored Affiliate Requests (formerly "Computer Account Requests"
  • Elevated VPN Requests
  • Secure Network Access Program Registration 

If you are a member of the Distributed IT or Group Sponsor communities and believe you should have access to submit these request types, you may request access by completing the form located on our TeamDynamix Ticketing Service page.
